Final Semester Grade Under "C" Form
To assist the Committee on Academic Standards in its review of student academic performance at the College, please complete this form for each student to whom you have awarded a C-, D, NC, or F as a final course grade.
Final Semester Incomplete Grade Form
The mark I (Incomplete) indicates that a small portion of the work of a course is unavoidably unfinished. This work must be completed before the end of the first week of the following semester including work for Semester II which must be completed before the end of the first week of the Summer Session. Other arrangements are authorized in writing by the Dean for Curriculum and Academic Engagement. If the work is not completed by the time specified, the I automatically becomes F or NC. Credit for a course completed at The College of Wooster will not normally be awarded after the deadline for changing incomplete grades, as indicated above.
Please assign a grade of I (Incomplete) only after you have conferred with the student. In order to assist in its review of student records, the Committee on Academic Standards requests that you complete this form regarding any I (Incomplete) grade you assign to a student and return it to the Office of the Dean of Students.
Interim Evaluation Form
Interim Evaluations are helpful in identifying students who are experiencing academic difficulty. Please complete this form for any student whose performance in your course is below a C or about whose academic work you have concerns. Interim Evaluations can be submitted at any time during the semester.
Student Grade Change Form
To change the final grade in a course, there must be a clear and compelling reason. Please complete this form to request a change in a final grade. This request should be made no later than the end of the second week of the following semester. Grade changes are normally approved due to a clerical error. A change of grade in a course taken at the College will not normally be permitted more than one semester after the date of completion of the course.
