The Ohio Department of Education and the Department of Education at the College of Wooster require students to keep track of the time spent preparing to become an educator. In addition, it is expected that you have ongoing evaluation-related discussions with your instructor. This is the purpose of this form.

These time sheets must be e-mailed to your instructor each week.

At the end of the semester, these times sheets must be completed, saved, and printed and turned in to your faculty member.

When responding, please consider your own questions and comments in addition to the following:

  • What went especially WELL?
  • What could you plan to IMPROVE next week, or work toward?
