Please note, the following email form is to be used to change name, address, telephone number or marital status. Copies of this notice will be forwarded to all appropriate offices by the Human Resources Office.

PLEASE NOTE: If you change your name, marital status and/or address, the Human Resources Staff strongly recommends that you come to Human Resources to complete new federal (W-4) and state tax forms. It is important that these forms accurately reflect your new name, marital status, and/or address.
