Student Employment Forms

Instructions

All employees of the College are required to complete the following paperwork on or before the first day of work:

  • Complete the information in the appropriate fields for each form. Use your legal name as it appears on your social security card. U.S. residents should complete all forms with home address; Non-U.S. residents should complete all forms with Wooster address.
  • Print each form and sign/date.
  • When all forms have been completed, give them to a Student Employment staff member along with your original forms of identification for Form I-9 verification. (Acceptable forms of identification are listed on page 9 of the Form I-9, Employment Eligibility Verification.)

Forms to be completed by all student employees who are U.S. citizens

  • Form I-9 (.pdf), Employmet Eligibility Verification - Complete Section 1 and present original form(s) of identification for Section 2. (Only print pages 7-8 of this form.) 
  • Form W-4 (.pdf), Employee Withholding Allowance Certificate (federal tax form) - Only print page 1 of this form.
  • Name and Social Security Number Verification Form (.pdf) - Complete this form using the information from your social security card.
  • Direct Deposit Form (.pdf) - The College requires all employees to have their net pay deposited into a checking or savings account. Students must supply the name of the bank, 9-digit routing number, and account number. It is recommended to submit a voided check or a deposit slip with the completed form.

Students with a permanent residence in Ohio

  • Form IT-4 (.pdf), Employee's Withholding Exemption Certificate (Ohio tax form)
  • Refer to the Ohio Public School District Numbers (.pdf) list or go to The Finder at tax.ohio.gov to identify your school district. Once you identify your school district, record the information in the fields marked Public School District of Residence and School District No. on the Form IT-4. 

Students with a permanent residence in a reciprocity state (IN, KY, MI, PA and WV)

  • Form IT-4NR (.pdf), Employee's Statement of Residency in a Reciprocity State 

Students with a permanent residence outside Ohio or a reciprocity state 

  • Form IT-4 (.pdf), Employee's Withholding Exemption Certificate (Ohio tax form) - Omit the fields marked Public School District of Residence and School District No. on the Form IT-4.

Students who are non-U.S. citizens

  • If you do not currently have a Social Security Number, please contact Jill Munro, Director of International Student and Scholar Services, so she can assist you with the application process.
  • Complete the following paperwork with your Wooster address:
    • Form I-9 (.pdf), Employmet Eligibility Verification - Complete Section 1 and present original form(s) of identification for Section 2. (Only print pages 7-8 of this form.)
    • Form IT-4 (.pdf), Employee's Withholding Exemption Certificate (Ohio tax form) - Indicate "Wooster" as the Public School District of Residence and "8510" as the School District No. on the Form IT-4.
    • Name and Social Security Number Verification Form (.pdf) - Complete this form using the information from your social security card.
    • Direct Deposit Form (.pdf) - The College requires all employees to have their net pay deposited into a checking or savings account.   • Schedule an appointment through Student Employment, after you receive your Social Security Number receipt (if applicable) to complete all of the required paperwork. Please complete the above paperwork to the best of your ability and bring it with you to the meeting. Also, please bring your passport, I-20 (or other immigration documents), and Social Security Number receipt with you to your appointment.
